This application must be completed and submitted by the parent or the legal guardian of the child applying for membership with Kids for Kids Junior. KFK uses the information you submit for the purpose of evaluating this application. In the event your child joins Kids for Kids Junior, this information will be used to facilitate the involvement of your child in KFK Junior, determine age-appropriate activities for your child as a member of KFF Junior, sending your child a birthday card to celebrate his or her birthday, and as otherwise set forth in our Privacy Policy. We recommend using the e-mail address of the parent or guardian, rather than of the child.

The Kids for Kids Junior Committee was established to raise money for children and families struggling with medical challenges, spread awareness, and build the next generation of young leaders in our community. We value hearing directly from our Junior Committee Applicants on what joining Kids for Kids will mean to them.
